Companies in specific industries with more than 10 employees are required to maintain injury statistics every year, using Forms 300, 300A and 301.  A set of forms must be completed for each physical location even if all locations are part of the same entity.

Injuries must be classified as death, days away from work, days of restricted work, and injuries that required a medical professional’s care and not just first aid.  As your Ohio BWC TPA, I can help since I have access to the medical records for each injury.

At the end of the calendar year, employers must enter the total number of employees and total hours worked for the year.  Every individual who worked at the company, including full time, part time, seasonal and temporary must be included.  An officer must sign the form.

Between February 1 and April 30, employers must post the Summary (Form 300A) with the other employment posters so employees can see them.  On May 1, take down the Summary and file it with Forms 300 and 301 for that year.  The forms must be kept for five years, and provided to an OSHA inspector upon request.

Data submitted to OSHA is considered confidential and will not be released to the public.
